If you would like to pre-order your CPDW 40th Anniversary cookbooks (and make sure that you get as many as you want), please fill out the form below and return it to CPDW no later than Friday, December 11th, 2009.  Cookbooks will be available at CPDW's Special Holiday Performance, Sunday, December 13th, 2009, and at the studio thereafter.

Each cookbook costs $8.00. You may pay with your order, or when you pick up your cookbooks. Checks should be made payable to CPDW.
